Piracetam is the prototypical racetam, and therefore needs no introduction.Tentative evidence suggests that piracetam treatment facilitates recovery from brain injury and has neuroprotective properties both in vitro and in vivo.Piracetam’s mechanism of action in the brain still remains poorly characterized despite decades of research. Paradoxically, piracetam appears to activate glutamate-dependent calcium entry into cells but does not exacerbate or induce excitotoxicity. Since piracteam positively modulates excitatory AMPA receptor currents, but is also an analogue of the inhibitory neurotransmitter GABA, it may be that piracetam enhances excitatory and inhibitory signaling concomitantly.Piracteam increases oxygen utilization in the brain, cell membrane and mitochondrial permeability, and the synthesis of cytochrome b5.A meta-analysis of 19 double-blind placebo-controlled studies on the use of piracetam found that patients with a variety of neurological disorders improved 60.9% with piracetam treatment as compared to controls.
